Registration Steps

  1. Complete and send the Registration Form.
  2. Pay for the program you have chosen.
  3. Complete, Sign and Print the Medical Release Form (link to the form) and Photography Release Form (link to the form). Mail the forms together with a copy of the Player’s Birth Certificate to: 11962 CR 101 - Suite 302 - PMB# 141 The Villages, Florida - 32162, or bring to the coach before the start of the program.

Your registration will NOT be complete and the Player will not be allowed to participate in any of the programs until all steps described above are taken to the satisfaction of TVSC.
